For those students attending the Cheley Outdoor Education field trip on October 7-10th, medications and Adams twelve medical forms were due to me last Friday. I will not accept any medications past this Wednesday (September 30th) unless you have an emergency situation. So far I have only received forms and medication from about ten students other than those that already have medication in the clinic. It is against the policies of the school district to send medications with your child unless you have filled out the appropriate forms and given the medication to me in its original container. Some people have turned in only the second page of the Adams twelve medical form. We need the first page and the second page. The first page contains information about the dosage, frequency, and reason for the medication along with a place for the doctor’s signature and office contact information. Do not send any medication in their luggage or backpack. Medications have to be handed to me by the parent and accompanied with the appropriate paperwork. This applies to over the counter medication as well as prescription medication. If your child is bringing an inhaler or medication that you cannot give me in advance please have it turned into me at 8a.m. the morning of their departure. (The paperwork for these medications must be turned in by this Wednesday September 30th.) Please be considerate about following the rules. I have an hour to get all the medications packed up and handed over to the teachers prior to their departure. It takes a great deal of time to prepare the spread sheets for medication administration along with other pertinent medical information for each student. I cannot do this in the hour prior to their departure nor do I have time to search through backpacks for medication. Thank you for your consideration concerning these issues.
