6.  Additional Information

a.  Financial Assistance for Testing:  Included in the Admissions Packet is the Family Economic Data Survey for Alternate Program Funding/Eligibility.  This form is an optional part of the Admissions packet.  Families may receive financial assistance with the IQ testing if they submit the packet and qualify for assistance as determined by Federal Free and Reduced Lunch eligibility guidelines.  The Governance Board designates a budget amount for this assistance annually, and these funds will be distributed on a first-applied, first-served basis until they are exhausted.

b.  Students with Special Needs:  Stargate School will not reject a student for admission on the basis that the student is currently on an Individualized Education Plan (IEP) as provided for by the Individuals with Disabilities Education Act (IDEA) or 504 Plan as provided for by the Americans with Disabilities Act (ADA), or on the basis of previously identified special needs, unless a review in cooperation with Adams District 12 Special Education Director shows that Stargate School would not be able to provide appropriate services without changing the fundamental nature of the Stargate program or creating other undue hardships.

The application packet includes a place for parent(s) to inform the school whether the student has previously identified special needs, including but not limited to a formal IEP or 504 plan.  Failure to provide accurate information may result in delay in receipt of needed educational services and may delay a final determination of a student's eligibility.

c.  Minimum Age:  Stargate School reserves the right to deny admission to applicants for Kindergarten who do not reach age five (5) by September 15th of the school year applied for, or for applicants to first grade who do not reach age six (6) by September 15th of the school year applied for.  The only exception is if an applicant applies through the Adams District 12's Early KG Admittance Program and is accepted through the program.  Accepted applicants will then have to participate in the lottery process. 

d.  Home-Schooled Applicants:  Students who are currently home-schooled are asked to find an adult who knows the student in an educational setting to complete the Teacher Nomination form.  When no other adult is available, the student may be invited for academic screening.

 e.  Applicants prior to 2010-2011:  For students placed on the wait list prior to the 2008-2009 application-year, the prior wait list policies will apply.  A student qualified for admission to Stargate, but placed on the wait list, may remain on the wait list each year, without requalifying.  Written notice must be received annually by the school to maintain placement on the wait list.  Wait listed students who do not reaffirm their desire to remain on the wait list will be purged from the list.

 f.  Applicants after 2009-2010:  For students applying for the upcoming school year and beyond, openings in any grade level will be distributed by lottery.  If an offer is not made to a student for the upcoming school year, the student returns to the lottery pool.  The parent(s) must annually reaffirm in writing the desire for the student to remain in the lottery pool or they will be removed.

g.  Admissions Deadline:  After the admissions deadline has passed, if there is space available and no students eligible for a lottery, Stargate will process applications and place students for grades 1-8 with a rolling admissions process so that a qualified student can be placed immediately.  Once the admissions deadline has passed, Stargate School will annually purge all incomplete applications.

h.  Equal Opportunity:  Stargate School will not deny any student admission on the basis of race, color, religion, sex, or national and ethnic origin.

i.  Return Policy:  Students who leave Stargate School for one year or less may reapply and will be placed on a wait list behind siblings of currently enrolled students.  After one year, students who reapply will enter the lottery process along with new applicants.  Students who have previously met nationally accepted IQ testing criteria will not need to re-test.

ADMISSIONS POLICY IS SUBJECT TO ANNUAL REVIEW
